Position Summary The Brant Community Healthcare System believes good healthcare is culturally safe and inclusive. For Indigenous patients this includes the understanding that supports and linkages for healing must be offered in parallel to the healthcare journey. Healing requires an environment that offers support from Indigenous professionals and staff, is open to listening and acting on truths that are shared and includes Indigenous ways of knowing/being/healing. We recognize and uphold an Indigenous and Western process of care and service delivery for Indigenous patients and families. It is within this context that the Indigenous Wellness Social Worker will be employed. The Indigenous Wellness Social Worker will work to ensure patient safety, clarity, and satisfaction in their hospital journey. This is accomplished by working collaboratively with various services such as nursing, hospitalists, social work, and patient relations staff with direct communication and partnership with the Indigenous Health Services Manager and Indigenous Medicine Physician Lead. Primary Responsibilities Patient Engagement and Advocacy Communicates with patients/family members patient’s care needs and triages/facilitates access to Indigenous Medicine for support in providing in-depth traditional, medical, and/or systems knowledge to promote a culturally safe and supportive environment and experience. Collaborate with Patient Relations Navigators/Social Work to provide ongoing social support to Indigenous patients and families. Liaise with patient’s MRP and/or Indigenous Medicine Physician Lead to support patient’s understanding of their medical journey, support informed decision making, and ensure culturally safe care. Facilitate the intake and follow-up of patient experience questions and concerns with the health care team to achieve resolution and where possible to avoid complaints process. Link with the Indigenous Health Manager and Indigenous Traditional Medicine Lead to ensure seamless care and supportive delivery of service in alignment with the Indigenous Health Strategy. Help address barriers such as systemic racism, mistrust of healthcare systems, and communication challenges. Help staff understand cultural protocols, family structures, and community considerations. Traditional Medicine Prepare traditional medicines for patients. Work to cultivate and provide Indigenous teachings for patients and families. Liaise with all Indigenous cultures to ensure we have a breadth of knowledge and respecting the diversity of Indigenous cultures. Collaboratively work with Indigenous Medicine Physician Lead to supply traditional medicine. Participate in reflexology, reiki, FIT, EFT, and other energy and spiritual work. Administration Document in HMS patient engagement, document and monitor patients. Organizational, planning, and time management skills, with the ability to multi-task, produce and coordinate projects to tight deadlines. Analytical and critical thinking skills including the ability to prepare and deliver presentations, as well as write plans, proposals, procedures, reports, and briefing material. Knowledge of Microsoft Office Suite (Word, Access, PowerPoint, and Excel). Perform administrative duties such as follow up phone calls, organizing and filing charts, recording data. Education & Training Facilitate traditional teachings to staff, patients, and families. Participate in hospital Indigenous health committees or reconciliation initiatives. Counselling and Clinical Social Work Services Provide crisis counselling Address mental health concerns, grief, trauma, addiction, and family stress. Use trauma-informed practice Support families during grief, illness, and/or child protection involvement provide emotional support through their stay at BCHS Helping patients understand treatment options Assess risk, safety, and social determinants of health of indigenous patients Mandatory Qualifications Applicant must be Indigenous Bachelor of Social Work (BSW) or Master of Social Work (MSW) from an accredited program required Registration in good standing with the Ontario College of Social Workers and Social Service Workers required 3-5 years of previous social work experience required. Experience working with First Nations, Inuit, and Métis communities required. Must have experience in providing traditional medicine as well as reflexology, reiki, FIT, EFT, and other energy and spiritual work. Experience in hospital or healthcare social work preferred.li> Knowledge of Indigenous health issues and social determinants of health.
